Withdrawing for personal reasons\u00a0does not require you to provide proof of your\u00a0medical challenges.<\/p>\n<h2>Medical Withdrawal<\/h2>\n<p style=\"padding-left: 30px;\">To request a withdrawal for medical reasons,\u00a0you submit paperwork so that University\u00a0Health Services can assess your condition\u00a0and request a withdrawal on your behalf.\u00a0College deans and advisors prefer you to\u00a0request a medical withdrawal when\u00a0appropriate so that they know you are getting\u00a0proper treatment and are ready to pursue your\u00a0studies when you return. In addition, only Tang\u00a0can back date a withdrawal to the time you\u00a0became ill, which can be important for fee\u00a0adjustments.<\/p>\n<p style=\"padding-left: 30px;\">See the bottom of this page at &#8220;HOW TO REQUEST A MEDICAL WITHDRAWAL FROM UHS&#8221; for a step-by-step process.<\/p>\n<hr \/>\n<h3><strong>Can I avoid withdrawing?<\/strong><\/h3>\n<p style=\"padding-left: 30px;\">If possible, you should explore options other\u00a0than withdrawing. Withdrawal from the\u00a0university can bring up unforeseen issues with\u00a0your Financial Aid, housing, university fees,\u00a0visa\/immigration status, or SHIP Coverage.<\/p>\n<p style=\"padding-left: 30px;\">Consider talking to your academic advisor\u00a0to explore options such as reduced course load.\u00a0The <span style=\"text-decoration: underline;\"><strong><a href=\"http:\/\/dsp.berkeley.edu\/\">Disabled Students Program (DSP)<\/a><\/strong><\/span> is\u00a0authorized to offer a range of specific\u00a0accommodations to make classes\u00a0accessible to students with disabilities.<\/p>\n<hr \/>\n<h3><strong>Important Contacts<\/strong><\/h3>\n<h4><span style=\"color: #000000;\"><em>Cal Student Central<\/em><\/span><\/h4>\n<p style=\"padding-left: 30px;\">It is important to speak with a financial aid advisor at <span style=\"text-decoration: underline;\"><strong><a href=\"http:\/\/studentcentral.berkeley.edu\/contact\">Cal Student Central<\/a><\/strong><\/span> before withdrawing from the university for medical reasons. You will be held responsible for all Housing and Dining charges prior to the first day of courses and after your effective date of withdrawal, if you are still in student housing.<\/p>\n<p>Fee adjustments, if any, are based on your withdrawal date. All fee adjustments for withdrawals are applied to your student account and may be applied to any outstanding debts. If you are participating in the Deferred Payment Plan and have not paid all your fees, you may still owe fees to the University. Please see tables below for prorated amounts.<\/p>\n<p>These fees are nonrefundable:<\/p>\n<ul>\n<li>late fees<\/li>\n<li>health insurance<\/li>\n<li>class pass<\/li>\n<li>document management<\/li>\n<\/ul>\n<hr \/>\n<h3><strong>How to Request a Medical\u00a0Withdrawal from UHS<\/strong><\/h3>\n<p>In order to have your medical withdrawal\u00a0approved, you need the Social Services (SOS)\u00a0office at the Tang Center to verify your medical\u00a0reasons for withdrawing.<\/p>\n<p>Medical withdrawals are considered for the current or previous semester. To be eligible for a medical withdrawal you must have registered and paid a percentage of your fees for the semester.<\/p>\n<h4><strong>1. Send paperwork to SOS.<\/strong><\/h4>\n<p style=\"padding-left: 30px;\">Information Required (See the <span style=\"text-decoration: underline;\"><strong><a href=\"https:\/\/uhs.berkeley.edu\/sites\/default\/files\/checklist_medical_withdrawals.pdf\">Checklist for Medical Withdrawals<\/a>)<\/strong><\/span><\/p>\n<ul>\n<li>Submit a <span style=\"text-decoration: underline;\"><strong><a href=\"https:\/\/uhs.berkeley.edu\/sites\/default\/files\/notice_of_withdrawal_0.pdf\">Notice of Withdrawal form<\/a><\/strong><\/span><\/li>\n<li>Detailed medical record information (including diagnosis and treatment)<\/li>\n<li>Your UHS medical record may be reviewed with a <span style=\"text-decoration: underline;\"><strong><a href=\"https:\/\/uhs.berkeley.edu\/sites\/default\/files\/roiforweb.pdf\">Release of Information Form<\/a><\/strong><\/span><\/li>\n<li>If you were treated in the community or at the UHS Counseling &amp; Psychological Services, request that a <span style=\"text-decoration: underline;\"><strong><a href=\"https:\/\/uhs.berkeley.edu\/sites\/default\/files\/health_eval_withdrawal.pdf\">Health Evaluation Form<\/a><\/strong><\/span> be completed by your provider. Your outside medical records can be reviewed with this Release of Information form.<\/li>\n<li>You may also submit a brief explanation (up to one page) of the reasons you are requesting a medical withdrawal.<\/li>\n<\/ul>\n<p style=\"padding-left: 30px;\">Medical Withdrawal and Readmission Forms are also available in-person at the Social Services office.<\/p>\n<p style=\"padding-left: 30px;\">All information should be sent to Social Services, along with a copy of the Release of Information form you sign, so your provider can communicate with us.<\/p>\n<p style=\"padding-left: 30px;\">The above information is reviewed to determine if the medical condition warrants a medical withdrawal. The withdrawal determination is based on the severity and onset of your condition and its impact on your ability to attend classes over a period of time. The medical withdrawal documentation is also reviewed to determine what, if any, medical documentation will be necessary for health clearance when you seek to return to school. It is common for continued treatment over a period of time to be required as a condition of clearance.<\/p>\n<h4><strong>2. Wait for confirmation from SOS.<\/strong><\/h4>\n<p style=\"padding-left: 30px;\">As soon as Social Services has made a\u00a0decision, they will try to contact you by phone.\u00a0Social Services takes care of contacting your\u00a0college and the registrar on your behalf, so this\u00a0is the final step before you can leave the\u00a0university.<\/p>\n<p style=\"padding-left: 30px;\">They will also send a letter providing\u00a0information about how you can return to the\u00a0university when ready. Depending on your\u00a0situation, the letter may also have expectations\u00a0for your treatment.<\/p>\n<hr \/>\n<h3><strong>How to Return to Cal After a Medical Withdrawal<\/strong><\/h3>\n<p>The university will in some cases place a\u00a0\u201cmedical block\u201d on your registration. This will\u00a0prevent you from re-registering (except for\u00a0Summer sessions) until you receive health\u00a0clearance from UHS.<\/p>\n<h4><strong>1. Clear your medical block, if you have one.<\/strong><\/h4>\n<p style=\"padding-left: 30px;\">Fill out a \u201cHealth Clearance Application&#8221;.\u00a0You may need to fill out another \u201cRelease of\u00a0Information\u201d form to allow your treatment\u00a0provider(s) to share information with the\u00a0Medical Withdrawal Committee.\u00a0Ask your current healthcare provider to fill out\u00a0a \u201cHealth Evaluation Form\u201d and send it to\u00a0SOS by mail or fax like before.<\/p>\n<h4><strong>2.
